 Death Claim Application - Form VIII

Completion of death claim application - Form VIII is mandatory when applying for benefits of a deceased member.

If the deceased member has worked in several institutions, has been absorbed into the pension status in a local government institution or if several applicants are applying for benefits, the following applications should also be completed.

 Death Claim Application - Form VIII-A
 (To be completed by other applicants)
 Certificate Of Employer For Deceased Member - Form VIII-B
 Form VIII-C
 (To be completed by the Employer for the employees who were
 absorbed into the pension scheme of the local government service)
