After claiming benefits upon leaving an institution, a member can make another claim only after five years have elapsed from the date of termination of employment from the institution for which the previous claim was made.
The following circumstances are considered exceptions, allowing members to claim benefits without applying the 5-year rule:
01. If you require your payment urgently, carefully read the instruction leaflet attached to the application or available online before completing the benefit application form. The application must be completed accurately and clearly in legible handwriting.
02. Submit the application only after the employer has credited contributions to our Board up to the last month of employment.
03. Benefit applications may be forwarded to us through the employer. Applications sent personally by post will not be accepted. When submitting applications in person, the applicant’s presence at the Head Office or Regional Offices is mandatory.
04. I. If you have worked in several institutions, the application should be completed through the last employer.
II. If benefits are to be claimed for previous institutions, one or more of the following documents must be attached as proof of employment:
If such documents are unavailable, separate applications should be completed for those institutions.
III. Information such as employment commencement and termination dates, employer number, and member number must be included on the first page of the application.
IV. A clear photocopy of the National Identity Card certified by the employer must be submitted with the application.
V. Details of an active bank account opened in the applicant’s name must be correctly stated in the application.
VI. A clear copy of the bank passbook or bank statement must be submitted with the application.
VII. The bank name, branch, account number, and applicant’s name must be clearly visible on the photocopy.
VIII. If a bank passbook is not used, the bank details mentioned in the application must be certified as correct by the relevant bank before submission.
05. If the institutions where you worked have been closed down, the member must complete Part (1) of the benefit application and submit it together with:
• Indemnity (VI – D)
• Personal information form (VI -C)
• Copy of National Identity Card
These documents must be certified by the Grama Niladhari and Divisional Secretary of the applicant’s residential area. Proof of employment documents must also be submitted compulsorily.
1. Foreign Migration (Passport, visa, and air ticket must be submitted.)
2. Medical Reasons (Relevant medical reports must be submitted.)
3. Redemption of Pawned Gold Items (Pawn receipts and a request letter must be submitted.)
4. Marriage of a Member’s Child (Request letter and wedding invitation card must be submitted.)
5. Educational Expenses (A letter from the relevant educational institution regarding the payment and the applicant’s request letter must be submitted.)
Under such special circumstances, benefit applications are accepted through a special procedure together with supporting documents, and payments are made within 4 working days.
Benefit payments are carried out through our Head Office as well as the Regional Offices in Gampaha, Kandy, Kurunegala, Matara, Ratnapura, Badulla, and Nuwara Eliya.
